Provider Resources

Florence Wormald Heart & Vascular Institute

St. Elizabeth proudly partners with referring providers across Northern Kentucky, Southeast Indiana and the Greater Cincinnati region to facilitate timely cardiovascular evaluation and specialty access.

This page provides a centralized overview of referral pathways, access options and decision-support resources to guide appropriate escalation of care.

Clinical Referral Algorithms

These downloadable algorithms are intended for use in the outpatient setting and summarize common clinical inflection points, suggested preliminary evaluation and logical next steps for select cardiovascular conditions.

They are designed as practical aids to support day‑to‑day decision‑making and are not intended to replace clinical judgment or formal guidelines.

Expedited Evaluation & Second Opinions

Expedited access options are available for patients with:

  • Rapidly progressive or unstable cardiovascular disease.
  • Complex or high‑risk conditions requiring multidisciplinary input.
  • Conflicting recommendations or uncertain next steps.

Referring physicians are encouraged to indicate urgency at the time of referral or contact program offices directly when time‑sensitive evaluation is needed.

What Happens After Referral

Once a patient is referred, our teams support coordinated evaluation through:

  • Intake and triage aligned to the patient’s clinical needs.
  • Multidisciplinary review when appropriate.
  • Ongoing communication with the referring physician throughout evaluation and treatment planning.

Clinical Research & Specialized Evaluation

Patients may be referred for research evaluation without identifying a specific trial. Research staff assist with study matching and coordination.
