The 80-hour course includes theory, as well as supervised lab and clinical training. Upon completion of the course, students are qualified to take the competency evaluation (State Test) at a recognized state testing center. Thomas, KY 41075 There are two types of courses being offered in 2021, Option A or Option B details are listed below including course dates and times. Option A: State-Tested Nurse Aide Training 10-Day Course The course combines classroom and clinical time during a 10-day period to complete the course requirements. The 2021 courses will be posted on our career website as paid training for a CNA Applicant approximately six-eight weeks prior to each course start date. The application for the CNA Applicant position is accessed and submitted online through our career website. A work agreement must be signed in order to participate. After review, some applicants may receive a phone and/or in-person interview with the possibility of future employment and paid training. For current associates interested in applying for the paid training courses, you would be applying for a transfer from your current department. The transfer period is four weeks. The online course combines computer-based learning modules and on-site clinical time to complete course requirements. All students must show proof of acceptance to college or university. Schedule an orientation six weeks prior to course start date to review course expectations and online assignments. The cost is $595 for non-employees. All 45 computer modules and workbook chapters must be complete prior to attending the 4 clinical dates. For CNA Applicants, some requirements are completed upon hiring. NO artificial nails of any kind are permitted in patient care areas. Application and $100 deposit required to hold a spot in the online course (or on waiting list, if applicable). Most classes fill up several weeks in advance of the start date. Please call or email coordinator (contact information listed above) to confirm there is an open spot available in the online course before sending documents. Payment in full is needed on the orientation date for the online course. Additional Prerequisites (needed at least two weeks in advance of class-specific dates TBA): Proof of high school diploma, GED, college transcripts/acceptance letter or equivalent. Proof of Health Insurance. Physical within the last 12 months stating you can participate in the class. Two step PPD skin test - both read as negative or a Blood Assay for Mycobacterium tuberculosis (BMAT) test - read as negative (If either test is positive, will need a chest x-ray confirming no TB). The initial PPD test shall count as the second step PPD if medical documentation of the one step PPD was negative within one year prior to the initial test. MMR (documentation if born after 1957) – or titers drawn showing levels to be within normal limits. Hepatitis B vaccination records OR proof of starting vaccination OR signed waiver. Influenza Vaccination Proof of Liability Insurance – will give information on after enrolled in class (cost is generally around $45). Proof of results of criminal record report(s) for KY (courts.ky.gov, approx. $27) and state of residency (if they differ). At time of Interview and Orientation Participants must be able to commit to the classroom and clinical time requirements described in the course syllabus. Information will be given regarding uniform for clinicals, dress code for class, and class expectations. Students with incomplete paperwork will not be permitted to start class.
