To request your medical record on MyChart, follow these instructions Log into MyChart and scroll over to the Messaging tab on the top of the screen. From the tabulated list, click on Request your Hospital Record Complete all fields on the page and click Submit Request MyChart requests are only authorized to upload your medical records into your individual portal. You must call the Radiology department at (859) 301-2160 to obtain your Radiology images and X-Ray film. How long does it take to get my records released? Medical record requests typically take less than two weeks to complete. How much does it cost to get a copy of your medical records? Under KRS 422.317, the first copy of your medical records is free. However, a second paper copy is available for $1 per page or $25 for a CD. Can you fax or email my medical records to me? No, we do not fax or email medical records. The only way to receive an electronic copy of your medical record is through MyChart. How can I get a copy of my bill? To obtain a copy of your hospital bill, call Patient Finance at (859) 655-4100. Can I request and download my requests through the mobile app? No, you are unable to request or download your medical records through the app.
