MyChart

MyChart provides a free patient portal for patients to access their medical records and stay connected electronically. This online tool allows you to manage your healthcare in a convenient and secure manner from your computer, tablet, and mobile device.

Requesting Medical Records

Option 1 – MyChart Request

Patients are able to access all hospital medical records through MyChart. To request your medical record on MyChart, follow these instructions

  1. Log into MyChart and scroll over to the Menu tab on the top of the screen.
  2. From the tabulated list, click on Request your Hospital Record
  3. Complete all fields on the page and click Submit Request

MyChart requests are only authorized to upload your medical records into your individual portal. For further questions, please contact the HIM department.

Option 2 – Release of Information Authorization Form

For medical record requests from Kentucky Providers and Hospitals:

Complete the AUTHORIZATION FOR USE OR DISCLOSURE OF PROTECTED HEALTH INFORMATION form. Once completed and signed, submit the form one of the following ways.

If you would like to pick up your medical records at the Edgewood location, please call ahead at (859) 301-3876. The HIM Department is open from 8:00 a.m. – 4:00 p.m., Monday through Friday.

For medical record requests from Indiana Providers and Dearborn Hospital:
If you would like to pick up your medical records at the Dearborn location, please call ahead at
(812) 496-7350. The Medical Records department is open from 8:00 a.m. – 4:30 p.m., Monday through Friday.

You must call the Radiology department at (859) 301-2160 to obtain your Radiology images and X-Ray film.

Medical record requests typically take less than two weeks to complete.

Under KRS 422.317, the first copy of your medical records is free. However, a second paper copy is available for $1 per page or $25 for a CD.

No, we do not fax or email medical records. The only way to receive an electronic copy of your medical record is through MyChart.

To obtain a copy of your hospital bill, call Customer Service at (877) 424-5750.

No, you are unable to request or download your medical records through the app.
