The St. Elizabeth Family Practice Center is dedicated to providing you with quality, compassionate health services and look forward to a successful partnership with you and your family for many years to come.

The Family Practice Center is a full service doctor's office with 24 resident physicians and a panel of faculty members. A resident physician is a graduate of a medical school with an M.D. or D.O. degree and is furthering his/her medical practice with three years of training in the specialty of Family Practice. We are an academic office where our patients receive state of the art medical care guided by Board Certified full-time faculty.

Our Services:
The Family Practice Center offers a broad scope of services including obstetrics, pediatrics, adolescent medicine, adult, geriatric, behavioral health counseling and holistic medicine.

We offer limited lab testing onsite as well as basic x-ray examinations. Procedures such as colposcopy, flexible sigmoidoscopy, nasopharyngoscopy, acupuncture and other primary care procedures are provided in our office.

Family Practice Center

All inpatient and outpatient care is managed by your selected physician. Our patients often tell us how much they appreciate the great access they have to care at our office. Patients with acurate or urgent problems are seen the same day they call.

Testing & Results:
We believe laboratory tests should only be done when necessary to establish a diagnosis, prevent illness or guide treatment. Your doctor will discuss with you any testing needed so you can participate in the decision.

• Test results are usually available within 2 weeks.
• We will call you with any results that need your attention sooner.
• If by chance, your results are not received in 2 weeks, please inform the office.
• Remember to keep your phone number, address and insurance information updated at our front desk.

Medication Refills:

• Please bring all your current medication bottles to each and every visit.
• Call your pharmacy for any refill requests, and they will contact us.
• Please allow at least 24 hours for our office to approve the refill.
• Some medications will not be refilled without an office appointment.
• No controlled substances will be prescribed or refilled after hours.

Billing Information:
For your convenience, Visa, MasterCard and Discover are accepted. Office co-payments are expected at the time of your office visit. We will be glad to submit your insurance forms if we have accurate and current insurance information from you.

Insurance providers do not pay for certain services, or may make only partial payments. You are responsible for payment of any charges not covered by your insurance plan. If financial circumstances requiring special attention arise, please disucss the problem with our billing office or call (859) 301-3810. We can arrange a mutually agreeable payment plan for you.

Office Hours & Appointments:
Our office and front desk hours are Monday thru Friday from 8 a.m. - 5 p.m. and Saturday 8 a.m. to noon. Patients are seen by appointment only. To schedule an appointment please call (859) 301-3800.

On Sundays, holidays and after hours, a doctor is always available by telephone. If you have a problem or question that cannot wait until regular office hours call us at (859) 301-3800. When our office is closed, we have a recording directing you to contact the doctor on-call. If your condition requires immediate attention or you feel it is life threatening, call 911 or go directly to the emergency department and the doctor will be notified after you arrive.
