The Cardiac Heart Failure Program was designed to improve long and short term outcomes for patients who have been diagnosed with heart failure. Our focus and success are in helping our heart failure patients proactively manage their heart disease, feel better faster and improve patient’s quality of life.
For your first visit with us, please bring a list of the medications you are currently taking, including vitamins and supplements. It is also important to wear comfortable clothing and shoes as you will undergo a brief walking test. We are located at St. Elizabeth South unit, the Cardiology department which is located on the first floor, adjacent to elevator D.
As a patient in our heart failure program, each visit you will be seen by an Allied Health Professional, that will also be available to answer your questions during times between your visits. During each visit, a brief physical exam will occur, and you will have the opportunity to discuss any changes in your symptoms. If your symptoms have changed, we may adjust your medications. As your condition improves, we will evaluate and adjust the frequency of visits needed.
At each visit, we will provide you and your family members with information about your medications, heart failure symptoms, diet, and activity. We will also provide you with the opportunity to speak with a dietician, pharmacist, or social worker if needed. After each visit with us, we will notify your referring physician and primary care physician of your progress.
